All employees who work a minimum of 30 hours per week are eligible for coverage on the 1st day of the month following 30 days of employment.

Summary of Benefits and Coverages

Aspire Living & Learning offers benefit eligible employees the choice between three medical plans – 2 copay plans and a high deductible health plan (HDHP). These plans offer varying levels of annual deductibles and out-of-pocket maximums so you can decide which plan best fits the needs of you and your family members. Employees who enroll in a medical plan are automatically enrolled in the Cigna vision plan at no additional cost.

The chart below is only a summary of the covered services. Out of network costs will vary.

If you are enrolled in the HDHP Bronze plan, you are eligible to independently open a Health Savings Account (HSA) at a bank of your choice. For more information on how to set up an HSA please visit: https://www.healthcare.gov/high-deductible-health-plan/setting-up-hsa/

Telehealth Services

Employees can access telehealth services to treat minor medical conditions like colds, flus, sore throats, allergies and more. Cigna’s telehealth services allow employees to connect with a board-certified doctor via video or phone. Telehealth is included in both medical plans. Appointments have no copay for members. Preventive Care

As we focus on helping employees maintain good health, we offer preventive care services covered at 100% with no deductibles, copays or coinsurance. Examples of preventive covered services include (but are not limited to): mammograms, colonoscopies, certain labs, immunizations, well-child care, and physicals for adults. Mental Health Resources

Employees can access a variety of virtual mental health resources through Cigna. These services are available with all three medical plans. The cost depends on the service or program you are utilizing.

Employee Assistance Program

An Employee Assistance Program (EAP) is offered to all Aspire employees and immediate family members through ACI Specialty Benefits. This is a completely confidential counseling program that covers issues such as marital and family concerns, depression, substance abuse, grief and loss, child or elder care referrals, legal consultations, financial assistance or other personal stressors.

Talkspace

  • The Talkspace app provides virtual access to licensed therapists via live video and private texting, plus psychiatrist services and additional resources.
  • Copay/coinsurance and deductible apply. Services are submitted and coded as a claim, per occurrence. Text-based therapy is billed per 60 minutes, on average once per week.

Ginger

  • Ginger virtually connects members with a certified coach via texting and app-based programs to help them manage anxiety, depression and daily stressors 24/7/365. If needed, a coach can add a licensed therapist or psychiatrist to the care team within days.

  • MDLIVE allows users to talk to a therapist or psychiatrist via video only (for behavioral services). Users do not gain direct access to a provider; they need to schedule a day/time for their appointment.
  • There is no cost if you are on the Copay Gold or Silver plan. There is no cost on the HDHP Bronze plan once you have met your deductible. Services are submitted & coded as a claim, per occurrence.

Telehealth Services

Employees can access telehealth services to treat minor medical conditions such as colds, flus, sore throats, allergies and more. Cigna’s Telehealth Connection allows employees to connect with a board-certified doctor via video or phone.

Telehealth is available with all three medical plans. Appointments have no employee cost if you are on the Copay Gold or Silver plans and no cost on the HDHP Bronze plan once you’ve met your deductible. Appointments can typically be scheduled within one hour or less. It is recommended that employees complete the telehealth registration process before services are needed to make scheduling an appointment quicker in the future.

Cigna's Diabetes Prevention Program (Omada)

Cigna’s Diabetes Prevention Program (Omada)

Cigna Diabetes Prevention Program in collaboration with Omada is a program to help you avoid the onset of diabetes, as well as health risks that might lead to heart disease or a stroke. The program is covered by your health plan at the preventive level, just like for your wellness visit. Program participants have access to a professional virtual health coach, an online support group, interactive lessons, and a smart-technology scale. The program will help you make small changes in your eating, activity, sleep, and stress to achieve healthy weight loss through a series of lessons and tools to help you maintain weight loss over time. You may also be offered the opportunity to join a gym for a low monthly fee and no enrollment fee.


There is no cost to the members who participate in this program and is highly recommended to members at risk for type 2 diabetes.

Flexible Spending Accounts

Eligibility

Flexible Spending Accounts

Flexible Spending Accounts provide you with an important tax advantage that can help pay healthcare & dependent care expenses on a pre-tax basis. By anticipating health care & dependent care costs for the next year & setting aside money, employees can lower their taxable income. FSA and DCA accounts are administered by Cigna.

*The IRS will release 2024 FSA maximum contributions in late October 2023. If increases to the maximum contribution are announced, the benefit enrollment system will be updated to reflect the accurate maximum amount that employees can contribute to their FSA annually.

Medical Flexible Spending Account (FSA)

The 2024 maximum an employee can contribute to an FSA is $3,200 per calendar year.

This program allows employees to use pre-tax dollars for IRS-approved expenses such as medical deductibles, copayments/coinsurance, prescription copays, hearing aids, & dental services. All participants will be issued a debit card to pay for eligible expenses or a claim form may be submitted.

Dependent Care Account (DCA)

The maximum amount an employee can contribute to a DCA is $5,000 (for single or married filing jointly) or $2,500 (if married and filing separately).

A DCA allows employees to use pre-tax dollars to fund qualified dependent care such as daycare for children under age 13 or elder care.

Northeast Delta Dental HOW Program

Delta Dental’s Health through Oral Wellness (HOW) program works with your existing dental benefits to help you achieve and maintain better oral wellness. Members who are at risk for dental decay or periodontal disease can request a clinical risk assessment preformed by participating dental providers. based on the results of the risk assessment, you may qualify for extra preventive services (like cleanings) not normally covered under the dental plan.

Basic & Voluntary Life Benefits

Summary of Benefits and Coverages

Aspire provides each benefit eligible employee with the core benefits of life insurance, and accidental death and dismemberment.

Below is an outline of the coverages available under each plan.

Group Basic Life and AD&D Insurance:

  • 100% employer funded
  • 2x earnings (rounding to the next higher $1,000), subject to a maximum of $500,000
  • Coverage amounts reduce by 35% at age 70 and by 50% at age 75
  • Your Basic AD&D coverage amount is equal to your Basic Life coverage amount

Supplemental Term Life and AD&D Insurance:

  • 100% employee funded
  • Provides coverage that you can take with you (portable) if you terminate employment
  • Employees may purchase coverage on themselves in increments of $10,000 to a maximum of $500,000
  • Employees may purchase coverage on their spouse in increments of $5,000 to a maximum of $250,000 (not to exceed 100% of the employee amount)
  • Employees may purchase from $2,000 to the maximum of $10,000 on their dependent children
  • Benefit eligible employees may enroll during the initial eligibility waiting period and during open enrollment each year
  • Employees may purchase up to $190,000 in supplemental term life insurance for themselves (or up to $25,000 for their dependent spouse) within the first 30 days of employment without having to complete a medical history statement (evidence of insurability form). If applying outside of the 30 days of employment, or for coverage in excess of the guarantee issue amounts, an evidence of insurability form will be required
  • AD&D coverage does not require evidence of insurability
  • In order to purchase supplemental term life insurance or AD&D coverage for your spouse and/or child, you must purchase coverage on yourself.

Long-Term Disability Insurance:

  • Benefit amount is 60% of covered monthly earnings to a maximum of $6,250 per month.
  • Benefits begin 180 consecutive days of total disability.

Short-Term Disability Insurance:

  • 100% funded by Aspire.
  • Benefit amount is 50% of covered weekly earnings.
  • Weekly benefit will be paid to you by your employer.
  • Benefits will begin on the 8th consecutive day of disability.
  • Benefits for one period of disability are paid up to a maximum of 25 weeks.

Accident & Critical Illness Insurance

Eligibility

Summary of Benefits and Coverages
Accident Insurance:

This benefit is voluntary and 100% Employee Funded.

Group Accident insurance is designed to help employees meet the out-of-pocket expenses and extra bills that can follow an accidental injury, whether minor or catastrophic. Lump sum benefits are paid directly to the covered employee if they sustain a covered injury.

Examples of lump sum benefits payable include: $100 for ambulance (ground); $150 for emergency room treatment; $100 for diagnostic examination.

Coverage can be elected for an employee, spouse or child. Benefits are paid on a tax-free basis; employee premiums are deducted on a post-tax basis.

Employee Bi-Weekly Premiums:

  • Employee Only = $4.73
  • Employee & Spouse = $7.73
  • Employee & Child(ren) = $9.81
  • Employee & Family = $12.92
Critical Illness Insurance:

This benefit is voluntary and 100% Employee Funded.

Group Critical Illness provides a fixed, lump-sum benefit upon diagnosis of a critical illness, which can include heart attack, stroke, paralysis, and more. These benefits are paid directly to the insured and may be used for any reason, from deductibles and prescriptions to transportation and child care.

Employees can purchase from a minimum of $5,000 to a maximum of $20,000 in coverage on themselves and/or their spouse. Spouse coverage cannot exceed 100% of the approved employee amount. Employees can also purchase 25% of the approved employee amount to a maximum of $5,000 to cover their dependent children.

Guarantee issue amounts are $20,000 for the employee coverage and $20,000 for the spouse coverage. Any coverage requested in excess amounts will require the completion of an evidence of insurability form.

Benefits are paid on a tax-free basis; employee premiums are deducted on a post-tax basis. Rates are based on age at the date of issue.
